EAYSO Electronic Registration

Go to EAYSO at WWW.EAYSO.ORG and follow the instructions below to help you through the process. EAYSO will open in a seperate screen for ease of use.

Important Instructions: Region 1343 requires that you enter your email address and telephone number on each online eayso registration. We use email to communicate important announcements regarding the season and upcoming events with our members when possible. We do not sell or share this information with anyone else.

If You Have Used EAYSO Before, Follow These Steps (otherwise scroll down to the “First Time” steps after this section)
• Click Log In to get to the Login Page and enter the email address and password you previously used
• The next screen is the General Instructions Page – click on “Player Form” to begin the pre-registration process. (If   you also wish to volunteer, please use the “Volunteer Form” link later on – all volunteers also need to pre-register and bring THREE printed forms in person to registration)
• Eayso takes you to the Region Selection page, click the first choice (I want my child(ren) to play in region) and   type in the number “1343” and click Next to proceed
• This brings up an information screen about the pre-registration process; read this and click Next
• The next screen begins the parent information screen, read and click next
• Complete or verify the parent infomation and click next
• Now review the volunteer position openings. Please choose at least one selection if at all possible. We will train you in all aspects of the selected position. We need your help and time to ensure that this program is a success for our children.
• The next screen provides a link to any players by name that you have registered before on eayso (it also provides a link for volunteers who have signed up before using eayso)
• Click the link with your player’s name and click Next, as necessary, to arrive at the Player Information screen
• This next screen begins the process of entering in the necessary registration information
• Enter in the requested Player information; red asterisks indicate REQUIRED fields; please note that we also require an email address and at least one telephone number to make contacting you convenient for our volunteers; click Submit when you are finished
• If adding another child click Add Another Player, otherwise click Print Forms and then click your player’s link by name on the following screen to print the forms. Also print any volunteer forms as required.
• The pre-registration form will come up and you should check that the information is correct
• Then, click on the “Click here to print” link at the top of this screen
• A windows print dialogue box will appear; enter in 2 copies and click Print

(A birth certificate is required for all new players to Region 1343)

• Mail in TWO COPIES of the registration form, payment (and birth certificate if required) to:
   AYSO Region 1343
   31 North 700 East # 256
   ST. George, Utah 84770

• On the Log In page click “First Time”
• This brings up an information screen to read, then click Next
• This brings up a screen with specific instructions about filling out the forms; read this and click Next
• The next screen will show three large buttons; click the left button “Ayso Parents” to pre-register your child
• On the next screen provide your name and phone number and click Next
• On the following screen type in your email address, a password and your secret question– these are REQUIRED – then click Next
• This brings up an information screen about the pre-registration process; read this and click Next
• EAYSO then takes you to the Region Selection page; click the first choice (I want my child(ren) to play in region)and type in the number “1343” (without the asterisks) and click Next to proceed
• This brings up an information screen about the pre-registration process; read this and click Next
• This brings up the Parent /Guardian Information screen, read this and click Next
• This next screen begins the process of entering in the necessary registration information
Enter in the requested Parent / Guardian information; red asterisks indicate REQUIRED fields; please note that we also require an email address and at least one telephone number to make contacting you convenient for our volunteers; click Next when you are finished
• The next screen asks if you wish to volunteer and provides choices you can select; please select one or more if you can volunteer and click Next, or otherwise simply proceed by clicking Next
• The next screen is the Player’s Information Page; enter in your child’s information, being sure to fill in all REQUIRED fields indicated by a red asterisk; click Next
• On the next screen, click on the “Print Forms” button, and then click your player’s link by name on the following screen to print the forms
• The pre-registration form will come up and you should check that the information is correct
• Then, click on the “Click here to print” link at the top of this screen
• A windows print dialogue box will appear; enter in 2 copies and click Print
